Vision Insurance with Provider Network

Provider Access and Choice

Our comprehensive networks include both private practice and retail store providers across the country. In fact, your current eye doctor may already be a member in our networks, meaning you can enjoy the greatest cost savings without making any care changes.

If you don’t already have an eye doctor, or your current doctor is not a network member, you may still choose to get your eye care outside of our networks. Just know that your out-of-pocket costs may be higher.

This choice gives you the greatest flexibility of selecting an eye doctor who meets your lifestyle, eye care and eye wear needs.

Eye Wear Choice and Value

Our vision benefits enable you to choose the prescription eyewear you prefer. Eyeglass lenses every year and eyeglass frames within your plan’s frame allowance every two years. Or, choose contact lenses every year instead of glasses.

Convenient Customer Service

In addition to our superior customer service team who are available by phone at (800) 657-8205, you have 24/7 access to your vision insurance information, see your claim status, and find general vision information at
