You can get vision insurance, underwritten by Golden Rule Insurance Company or UnitedHealthcare Life Insurance Company, in one of two ways: as a standalone plan or as a rider to your dental plan (vision plans are not available in all states).

Vision Insurance as a Standalone Plan

We are pleased to offer UnitedHealthcare Vision® plans for individuals and families in select states.1 We’ve “framed” our vision insurance plans to offer these benefits (among others):

  • Coverage choices – Plan A for glasses or contacts, Plan B for both glasses and contacts
  • A national network of providers for both services and eyewear you can use for the best coverage
  • The freedom to use a non-network provider2
  • No waiting periods
  • Coverage for people of all ages and at any stage of life3

You can visit to find providers in your area, access plan information, see claim status and more. Or call our superior customer service team at 1-800-273-8115 for more information. We think you’ll clearly see the value of vision insurance that can help you stay focused on the rest of your life.

Vision Insurance Rider with Dental Insurance

Vision Insurance as a Rider to Your Dental Plan

Keep an eye on your family's vision health by adding our optional Vision Benefit Rider4 to your dental insurance plan today. Vision benefits include routine eye exams, prescription glasses or contact lenses and discounts on other vision-related services (including laser surgery).

Our vision plans offer significant cost savings, a broad and diverse provider network and freedom of choice on eyewear. Take a look. We're sure you'll see the savings for yourself.

Provider Access and Choice

Our comprehensive network includes both private practice and retail store providers across the country. In fact, your current eye doctor may already be a member in our network, meaning you can enjoy the greatest cost savings without making any care changes.

If you don’t already have an eye doctor, or if your current doctor is not a network member, you may still choose to get your eye care outside of our networks. Just know that your out-of-pocket costs may be higher.

This choice gives you the greatest flexibility in selecting an eye doctor who meets your lifestyle, eye care and eye wear needs.

Convenient Customer Service

Our superior customer service is available when you need it. Representatives are available by phone at 1-800-657-8205. And at you have 24/7 access to your vision insurance information, your claim status and helpful vision information.

J.D. Power, a global marketing information company that represents the voice of the customer, ranked UnitedHealthcare Vision “Highest in Customer Satisfaction with Vision Plans, Three Years in a Row” in their J.D. Power 2013-2015 Vision Plan Satisfaction Reports.5
