You can get vision insurance in one of two ways: as a standalone plan or as a rider to your dental plan (vision plans are not available in all states).

Provider Access and Choice

Our comprehensive network includes both private practice and retail store providers across the country. In fact, your current eye doctor may already be a member in our network, meaning you can enjoy the greatest cost savings without making any care changes.

If you don’t already have an eye doctor, or if your current doctor is not a network member, you may still choose to get your eye care outside of our networks. Just know that your out-of-pocket costs may be higher.

This choice gives you the greatest flexibility in selecting an eye doctor who meets your lifestyle, eye care and eye wear needs.

Eye Wear Choice and Value

Our vision benefits enable you to choose the prescription eyewear you prefer. Eyeglass lenses every year and eyeglass frames within your plan's frame allowance every two years. Or, choose contact lenses every year instead of glasses.

5 You will receive a $130 retail frame allowance towards the purchase of any frame at a network provider.

6 You are eligible to select either eyeglasses (eyeglass lenses and/or eyeglass frames) or contact lenses, not both. Contacts chosen from the covered contact lens selection at a network provider. Non-selection contact lenses will receive an allowance. No copay applies to non-selection contact lenses.
